A telehealth nurse salary calculator turns your base rate, shift differential, certification pay and overtime into total yearly pay. BLS publishes no wage figure for telehealth nurses, so the tool compares your base rate with all registered nurses (SOC 29-1141). Their median wage was $46.90 an hour, or $97,550 a year, in May 2025.

How to Use the Telehealth Nurse Salary Calculator
- Pick hourly rate or annual salary. Type your base pay, your regular hours per week and the weeks you are paid in a year.
- Add the pay extras that apply to you: shift differential, certification or charge pay and overtime. Every extra comes from your own offer letter or pay stub. Leave a field blank if it does not apply.
- Read your total annual gross pay, then see where your base rate sits in the BLS wage table for all registered nurses.
What each result tells you:
| Result | What it means |
|---|---|
| Total annual gross pay | Base pay plus every extra you entered, before tax. |
| Base annual pay | Your straight-time rate times regular hours and weeks. |
| Pay extras | The yearly sum of shift differential, certification or charge pay and overtime and other annual pay. |
| Monthly, biweekly | Total annual gross pay divided by 12 and by 26. |
| Base hourly rate | Base pay per regular hour. Only this number is compared with BLS. |
| Average per hour worked | Total pay divided by regular plus overtime hours. |
| BLS band | The two published percentiles for all registered nurses that your base rate falls between. |
| Approximate percentile | A straight-line estimate between those two percentiles. BLS does not publish it. |
| Gap to BLS median | Your base rate minus the median hourly wage for all registered nurses. |
| Second offer rows | The second offer as annual and hourly base pay, the yearly difference and its percentile. |
The presets are examples, not typical wages. The first one is a day-shift triage nurse paid $44.00 an hour for 40 hours a week. That comes to $91,520.00 a year, about the 41st percentile for all registered nurses. BLS publishes no wage figure for telehealth nurses, so the page never shows a telehealth-only number.
What Is a Telehealth Nurse Salary?
A telehealth nurse salary is the gross yearly pay of a registered nurse who cares for patients by phone, video or other remote tools. It is base pay plus any shift differential, certification pay and overtime.
The American Academy of Ambulatory Care Nursing (AAACN) calls telehealth nursing an integral part of ambulatory care nursing. It began when registered nurses took patient calls and triaged callers to the right level of care. Today the work also runs on smartphones, kiosks and web platforms.
| Fact | What the official source says |
|---|---|
| BLS wage code | 29-1141 Registered Nurses. No separate telehealth series exists. |
| Core tasks (AAACN) | Assess needs with clinical algorithms, protocols or guidelines. Set the urgency. Build a care plan with the patient. Evaluate outcomes. |
| Where the work happens (AAACN) | A call center, a group practice, an after-hours service or a disease management program. |
| Licence | A state registered nurse licence. BLS says each state board of nursing sets the requirements. |
| Certification | No telehealth-only exam exists today. The Telephone Nursing Practice exam was discontinued in 2007. AAACN points telehealth nurses to the ambulatory care nursing certification exam, which includes telehealth content. |
| Jobs, all registered nurses | 3,465,400 in 2025, with about 180,800 openings a year projected through 2035. |
The practice facts come from AAACN pages read on 2026-10-02. The job figures come from the BLS Occupational Outlook Handbook. Telehealth nurses hold a registered nurse licence, so BLS counts them under code 29-1141 with all other registered nurses.
This tool is for nurses weighing a remote or call-center role. It is not a pay quote from any employer.
How Does the Telehealth Nurse Salary Calculator Work?
The calculator builds total pay from your base rate and your own extras, then places the base rate alone in the BLS wage distribution for all registered nurses.
Total annual pay = base rate x hours x weeks + pay extrasApproximate percentile = P1 + (base rate - W1) / (W2 - W1) x (P2 - P1)- Convert. An hourly rate is multiplied by regular hours and weeks. A salary is divided by them to get the base rate.
- Add shift differential. Differential pay = rate per hour x share of hours x paid hours in the year.
- Add certification or charge pay. Certification or charge pay = rate per hour x paid hours in the year.
- Add overtime. Overtime rate = (base rate + certification or charge pay + average differential per hour) x multiplier. Overtime pay = overtime hours per week x overtime rate x weeks. On-call standby pay is left out of the overtime rate.
- Find the band. The base rate is checked against the 10th, 25th, 50th, 75th and 90th percentile wages.
- Interpolate. W1 and W2 are the two BLS wages around your base rate. P1 and P2 are their percentiles.
The wage table below covers every registered nurse in the country, on site or remote. BLS does not publish a separate row for nurses who work by phone or video.
| Registered Nurses (SOC 29-1141) | Hourly wage | Annual wage |
|---|---|---|
| 10th percentile | $33.14 | $68,940 |
| 25th percentile | $38.62 | $80,330 |
| 50th percentile (median) | $46.90 | $97,550 |
| 75th percentile | $54.02 | $112,350 |
| 90th percentile | $66.09 | $137,470 |
| Mean (average) | $48.76 | $101,420 |

Telehealth Nurse Salary Example: Triage Nurse with Evening Shifts
A triage nurse works from home for a health system. She earns $45.75 an hour for 40 hours a week. Evening and weekend hours pay $3.00 more, and they make up 40% of her schedule. Her employer adds $1.00 an hour for her ambulatory care certification. She averages 3 overtime hours a week, and her offer lists a $600 yearly home-office stipend. A rival offer is $47.50 an hour.
| Step | Calculation | Result |
|---|---|---|
| Base annual pay | 45.75 x 40 x 52 | $95,160.00 |
| Shift differential | 3.00 x 40% x 2,080 hours | $2,496.00 |
| Certification pay | 1.00 x 2,080 hours | $2,080.00 |
| Overtime rate | (45.75 + 1.00 + 3.00 x 40%) x 1.5 | $71.93 |
| Overtime pay | 3 x 71.93 x 52 | $11,221.08 |
| Other annual pay | Stipend from the offer letter | $600.00 |
| Total annual gross pay | Base plus all extras | $111,557.08 |
| Average per hour worked | 111,557.08 / (43 x 52) | $49.89 |
| Approximate percentile, all registered nurses | 25 + (45.75 - 38.62) / (46.90 - 38.62) x 25 | About the 47th |
| Second offer | 47.50 x 40 x 52 | $98,800.00, +$3,640.00 a year |
Her base rate is $1.15 an hour under the median for all registered nurses. Her extras add $16,397.08 a year, so her average pay per hour worked is $49.89. The rival offer lifts base pay by $3,640.00 a year and lands near the 52nd percentile.
Factors That Change Telehealth Nurse Pay
Shift Coverage
After-hours work can add a large extra. AAACN lists after-hours services among telehealth settings. BLS notes that nurses in offices that do not give 24-hour care are more likely to work regular business hours. A $3.00 differential on a quarter of a full-time year adds $1,560.00. On every hour it adds $6,240.00.
State
State averages for registered nurses differ widely. The mean annual wage is $150,280 in California and $101,420 nationally. A remote job does not remove that gap. Ask which location sets the pay scale.
| State | Annual mean wage |
|---|---|
| California | $150,280 |
| Hawaii | $124,340 |
| Oregon | $123,140 |
| Washington | $121,540 |
| Massachusetts | $117,960 |

Certification
BLS says nurse certification is usually voluntary, though some employers require it. Certification pay is an employer choice. Enter your own figure.
Hourly or Salaried
A salaried nurse is paid the same for a long week. Divide the salary by real hours to see the true rate. The salary to hourly calculator does that in one step.
The Overtime Rate
The Department of Labor states that the regular rate includes shift differentials. The tool follows that rule. It multiplies base rate plus certification pay plus the average differential per hour by your multiplier. Hospitals may also use an 8 and 80 system instead of the 40-hour week, so check which one your employer uses.
Overtime adds to the total but not to the BLS comparison. Use the overtime pay calculator for weeks with mixed rates.
Remote vs On-Site Nurse Pay: What BLS Can Show
BLS does not compare remote and on-site nurse pay. It publishes one table for all registered nurses and breaks it down by industry and by area, not by where the nurse sits.
| Question | What BLS publishes | What it does not publish |
|---|---|---|
| Pay level | Percentile wages for all registered nurses. The median is $97,550. | A wage for telehealth or remote nurses. |
| Setting | Median of $91,230 in ambulatory healthcare services and $100,220 in hospitals. | A split between phone, video and in-person work. |
| Location | State mean wages, from $150,280 in California downward. | Which state a remote employer pays by. |
| Premium pay | Nothing. Wages exclude overtime and shift differentials. | Typical differentials for evening or night triage. |

The five states with the most registered nurse jobs are California (338,940), Texas (271,380), Florida (229,940), New York (205,810) and Pennsylvania (146,520). Any claim that remote nurses earn a set percent more or less than bedside nurses does not come from BLS.
When to Use the Telehealth Nurse Salary Calculator
Leaving the Bedside for a Remote Role
Enter your current base rate and differential first. Then enter the remote offer. Compare the two totals, since a day-shift remote job may drop a night differential you have now.
Checking a Call-Center Offer
Type the offered rate. A base rate of $42.00 an hour is about the 35th percentile for all registered nurses.
Testing a Salaried Offer
Switch to annual salary. A $90,000 salary over 2,080 hours is $43.27 an hour, about the 39th percentile.

Common Mistakes with Telehealth Nurse Pay
1. Treating the Result as a Telehealth Benchmark
The comparison group is all registered nurses. It includes hospital, clinic, school and remote nurses together.
2. Applying the Differential to Every Hour
A differential may cover only some of your shifts. Set the share of hours honestly. At 100% a $3.00 differential adds $6,240.00 a year. At 25% it adds $1,560.00.
3. Folding the Differential into the Base Rate
BLS wages leave out shift differentials. If you type a night rate as base pay, your percentile is overstated.
4. Comparing a Remote Offer with Your Home-State Average
An employer may set pay by its own location, by yours or by a national scale. Check before you judge the offer against a state mean.
5. Counting a Stipend as Hourly Pay
A home-office stipend is a fixed yearly amount. Put it in the other annual pay field, not in the base rate.
Accuracy and Limitations
The pay arithmetic is exact for the numbers you enter. The percentile is an estimate, and it describes all registered nurses, not telehealth nurses alone.
What it calculates accurately
- Base, extra and total gross pay for the year from your own figures.
- Monthly and biweekly gross pay.
- The BLS percentile band of your base rate among all registered nurses.
- The yearly base pay difference between two offers on the same schedule.
- Shift differential, certification pay and overtime pay for the year.
What it does not account for
- Pay for telehealth nurses as a separate group. No official series exists.
- Federal, state and local tax, or any payroll deduction.
- Your state, metro area, employer or experience.
- Benefits such as health insurance, retirement match and paid leave.
- Remote-work location rules and multi-state licence costs.
BLS wages are straight-time gross pay. They exclude overtime pay and shift differentials. The tool therefore compares your base rate only and reports the extras apart.

Frequently Asked Questions About Telehealth Nurse Salaries
How much do telehealth nurses make an hour?
No official hourly figure exists for telehealth nurses. BLS counts them as registered nurses, whose median wage was $46.90 an hour in May 2025. The lowest 10 percent of all registered nurses earned less than $33.14 and the highest 10 percent earned more than $66.09.
Do telehealth nurses get paid less than bedside nurses?
BLS data cannot answer that, because it has no telehealth series. For all registered nurses, the median was $91,230 in ambulatory healthcare services and $100,220 in hospitals. Your own offer may sit above or below either figure.
Is there a telehealth nursing certification?
There is no telehealth-only certification exam today. AAACN states that the Telephone Nursing Practice exam was discontinued in 2007. It encourages telehealth nurses to take the ambulatory care nursing certification exam, which has included more telehealth content since April 2009.
What does a telehealth nurse do?
A telehealth nurse assesses patients remotely, sets the urgency of their needs, builds a care plan with them and evaluates the outcome. AAACN says telehealth nurses use clinical algorithms, protocols or guidelines to do this.
Does working from home change my BLS percentile?
No. The percentile depends only on your base hourly rate. BLS does not split registered nurse wages by remote or on-site work, so the same wage table applies to both.
How much does a 3 dollar night differential add to yearly pay?
It adds $6,240.00 a year if every one of 2,080 paid hours earns it. If only a quarter of your hours earn it, the differential adds $1,560.00 a year.
Which state pays registered nurses the most?
California has the highest mean annual wage for registered nurses at $150,280 in May 2025. Hawaii follows at $124,340. These figures cover all registered nurses, not telehealth nurses alone.
Should I enter my night rate or my base rate?
Enter the base rate. Put the night or weekend add-on in the differential field. BLS wages exclude shift differentials, so a fair comparison uses the straight-time rate.
Can the calculator show my take-home pay?
No. It shows gross pay before tax and deductions. Take-home pay depends on your filing status, state and benefits, which this tool does not model.
Does the calculator store what I type?
No. Everything is worked out in your browser. The Save button keeps a result on your own device only, and nothing is sent to a server.
