OR Nurse Salary Calculator

Quick answer

An OR nurse salary calculator adds your base pay, on-call standby pay, call-back pay, certification pay and overtime into total annual gross pay. BLS publishes no wage figure for operating room nurses alone. It counts them as registered nurses, code 29-1141, with a median wage of $46.90 an hour, or $97,550 a year, in May 2025.

How to Use the OR Nurse Salary Calculator

  1. Choose hourly or annual pay. Enter your base pay, your regular hours per week and your weeks per year.
  2. Add your weekly on-call hours and on-call rate. Then add call-back hours, certification pay and overtime if you have them.
  3. Read your total annual gross pay, the breakdown table and where your base rate sits among all registered nurses.

What each result tells you:

ResultWhat it means
Total annual gross payBase pay plus every extra pay component you entered, before tax.
Base annual payStraight-time pay for your regular hours. No differentials, call pay or overtime.
Extra pay a yearThe sum of the extra components. The breakdown table lists each one.
On-call standby rowStandby hours times your on-call rate, for the weeks you entered.
Call-back rowCall-back hours times base rate times your multiplier.
Certification or charge pay rowExtra dollars per hour times all regular hours.
Overtime rowOvertime hours times the overtime rate. That rate is base plus certification pay plus the average differential, times your multiplier.
Monthly, biweekly, weeklyTotal annual gross pay divided by 12, 26 and 52.
Base hourly rateBase pay per regular hour. Only this number is compared with BLS.
BLS band and approximate percentileWhere your base rate falls among all registered nurses. It is not a ranking among operating room nurses.
Gap to RN medianYour base rate minus the BLS median hourly wage for all registered nurses.
Extra pay shareExtra pay as a percent of total pay.
Second offer rowsThe second offer as annual and hourly base pay, its percentile and the yearly difference.

The first preset is an example, not a typical operating room wage. It shows $46.00 an hour for 40 hours a week, 12 standby hours at $4.00, 2 call-back hours at 1.5 times and $1.00 of certification pay. That totals $107,432.00 a year. The $46.00 base rate sits near the 47th percentile for all registered nurses.

What Is an OR Nurse Salary?

An OR nurse salary is the gross pay a registered nurse earns in a year for work in an operating room. It combines a straight-time base rate with pay for being on call and for coming back in for urgent cases.

Operating room nurses are also called perioperative nurses. The Competency and Credentialing Institute certifies them with the CNOR credential, which more than 40,000 nurses hold. Its exam outline shows the scope of the work.

BLS publishes no wage figure for operating room nurses. The federal O*NET database lists Operating Room Registered Nurse as a job title inside code 29-1141, Registered Nurses. Every BLS number on this page covers all 3,379,720 registered nurse jobs.

This tool is for perioperative nurses checking an offer or a call schedule. It is not a pay quote from any employer. For LPN and nurse practitioner tables, use the nurse salary calculator.

How Does the OR Nurse Salary Calculator Work?

The calculator builds annual pay from your own pay components. It then places your base hourly rate in the BLS wage distribution for all registered nurses.

Formula: Total gross pay = base pay + extra pay
Base pay = hourly rate x regular hours per week x weeks per year
On-call pay = standby hours x on-call rate x weeks
Call-back pay = call-back hours x base rate x multiplier x weeks
Certification pay = dollars per hour x regular hours
Overtime pay = overtime hours x (base + certification pay + average differential) x multiplier x weeks
Approximate percentile = P1 + (base rate - W1) / (W2 - W1) x (P2 - P1)
  1. Convert. An hourly rate is multiplied by regular hours and weeks. A salary is divided by them to get the base rate.
  2. Add each extra. Every component uses only the numbers you typed. A blank field counts as zero.
  3. Find the band. The base rate is checked against the 10th, 25th, 50th, 75th and 90th percentile wages for registered nurses.
  4. Interpolate. W1 and W2 are the two BLS wages around your rate. P1 and P2 are their percentiles.

On-call pay and call-back pay are separate lines. Standby hours earn the low on-call rate. Hours worked after a call-in earn the base rate times your multiplier.

BLS annual wages equal the hourly wage times 2,080 hours. BLS wages are straight-time pay. They exclude overtime pay and shift differentials, so the tool keeps those out of the comparison.

OR Nurse Salary Example: 40 Hours Plus Weekly Call

An operating room nurse earns a base rate of $47.25 an hour for four 10-hour shifts a week. She carries the call pager 16 hours a week at $4.00 an hour. She works 3 call-back hours a week at 1.5 times base. Her certification pays $1.00 an hour. A surgery center offers $50.00 an hour with no call. These figures are an example, not survey data.

Call adds $14,385.28 a year to her pay. The surgery center offer raises base pay by $5,720.00 but has no call pay. Her current total of $114,745.28 is $10,745.28 above the offer, in exchange for 16 standby hours a week.

Factors That Change OR Nurse Pay

How Often You Take Call

Call frequency drives the standby line. Enter a weekly average for the whole year. A nurse on call every other week enters half of her call-week hours. In the example, 8 standby hours and 1.5 call-back hours a week pay $7,192.64 a year, half of the $14,385.28 from weekly call.

The On-Call Rate

Your employer sets the standby rate. The tool uses the dollar figure you type. It has no built-in percentage of base pay.

Whether Standby Time Counts as Hours Worked

Federal rules treat the two kinds of call differently. The U.S. Department of Labor says an employee who must stay on call on the employer premises is working. An employee on call at home is, in most cases, not working. This tool pays standby hours at your on-call rate, not at the base rate.

Call-Back Hours and Multiplier

Call-back pay grows with every call-in. Three hours a week at $70.88 pay $11,057.28 a year. Check your policy for the multiplier and for any minimum paid hours per call-in.

Where Registered Nurses Work

General medical and surgical hospitals hold 56.3% of registered nurse jobs. Outpatient care centers hold 5.1%, and BLS reports an annual mean wage of $112,690 there for all registered nurses. The national mean is $101,420.

Overtime

Hours past your scheduled week are overtime hours. The overtime pay calculator works out a single week from your own hours.

On-Call Pay vs Call-Back Pay for OR Nurses

On-call pay rewards being available, and call-back pay rewards working. They use different rates, so the calculator keeps them on separate lines.

In this example 3 worked hours pay 3.3 times as much as 16 standby hours. A call schedule with few call-ins pays far less than one with many.

BLS data cannot compare operating room nurses with other registered nurses. It reports one wage distribution for the whole occupation.

When to Use the OR Nurse Salary Calculator

Pricing a Call Requirement

Enter the call hours in the job posting and the posted standby rate. The extra pay line shows what the call duty is worth in a year.

Comparing a Call Job with a No-Call Job

Divide yearly call pay by yearly regular hours. In the example, $14,385.28 over 2,080 hours equals $6.92 an hour. A no-call offer needs a base rate that much higher to match the total.

Weighing the CNOR Credential

The CNOR application costs $475 and includes the first exam attempt. A retake costs $175. The credential lasts 5 years. Enter the certification pay your employer offers to see the yearly return. Not every employer pays one.

Budgeting from the Result

The result is gross pay. Read the gross vs net income guide before you plan a household budget from it. The salary to hourly calculator converts a salary without the BLS comparison.

Common Mistakes with OR Nurse Pay

1. Multiplying Standby Hours by the Base Rate

Sixteen standby hours at a $47.25 base rate is not $756.00 a week. At a $4.00 on-call rate it is $64.00. Enter the on-call rate from your policy.

2. Entering Call Weeks as Every Week

A rotation of one call week in four is not 52 call weeks. Average the hours over the year. Forty standby hours every fourth week is 10 hours a week.

3. Adding Call Pay to the Base Rate Before the BLS Check

The BLS wage is straight-time pay. A total of $114,745.28 divided by 2,080 hours gives $55.17 an hour, which overstates the $47.25 base rate.

4. Reading the Percentile as an Operating Room Ranking

The percentile ranks your base rate among all registered nurses in every setting. BLS has no ranking for perioperative nurses alone.

5. Forgetting the Multiplier Is a Policy Choice

The 1.5 in the call-back field is an example default. Some policies pay a different multiple or a minimum number of hours. Replace it with your own terms.

Accuracy and Limitations

The pay arithmetic is exact for the numbers you enter. The percentile is an estimate, and it describes all registered nurses.

What it calculates accurately

  • Base, extra and total annual gross pay from your own figures.
  • Monthly, biweekly and weekly gross amounts.
  • The BLS percentile band for your base rate among all registered nurses.
  • The yearly base-pay difference between two offers on the same schedule.
  • On-call standby, call-back, certification and overtime pay for the hours you enter.

What it does not account for

  • Any pay difference between operating room nurses and other registered nurses. BLS does not measure it.
  • Federal, state and local tax, or any payroll deduction.
  • Your state, metro area, employer, union contract or experience.
  • Benefits such as health insurance, retirement match and paid leave.
  • Wages after May 2025. BLS updates the survey once a year.
  • Minimum paid hours per call-in. Enter the paid hours, not the worked hours, if your policy has a minimum.
  • Whether your standby time counts as hours worked under wage law.

Your pay stub is the final check. Payroll rules for overtime and call pay differ by employer and by state.

How We Calculate OR Nurse Pay and the RN Percentile

Method
Annual pay built from user-entered pay components. Percentile band and linear interpolation against BLS OEWS national percentile wages.
BLS data
OEWS national estimates, May 2025, for 29-1141 Registered Nurses. Read on 2026-10-02 in the BLS OEWS Profiles tool. The figures are fixed constants on this page.
Why registered nurses
BLS has no operating room nurse series. O*NET lists Operating Room Registered Nurse and Certified Operating Room Nurse as job titles under 29-1141.00, Registered Nurses.
Inputs used
Pay type, base pay, regular hours, weeks, on-call hours and rate, call-back hours and multiplier, certification or charge pay, overtime hours and multiplier, other annual pay and an optional second offer.
Assumptions
A BLS work year is 2,080 hours. Wages are spread evenly between two published percentiles. Weekly extras repeat for every week you enter. Monthly is annual / 12, biweekly is annual / 26 and weekly is annual / 52.
Rounding
Money to the cent. Overtime and call-back rates are rounded to the cent before they are multiplied. The base rate is rounded to the cent before the BLS comparison. Approximate percentile to a whole number. Percent values to one decimal.
Edge cases
A base rate below the 10th or above the 90th percentile gets a band but no percentile number. Negative values, a multiplier below 1, more than 168 hours a week and more than 52 weeks return an error.
Not included
Tax, cost of living, experience and specialty adjustments. No specialty premium or multiplier is built in.

Frequently Asked Questions About OR Nurse Salaries

How much do OR nurses make a year?

BLS does not publish a figure for operating room nurses. It counts them as registered nurses, whose median annual wage was $97,550 in May 2025. The lowest 10 percent of all registered nurses earned less than $68,940 and the highest 10 percent earned more than $137,470.

Is there a BLS code for operating room or perioperative nurses?

No. BLS has one code for registered nurses, 29-1141. The O*NET database lists Operating Room Registered Nurse and Certified Operating Room Nurse as job titles inside that code, so their wages are part of the registered nurse figures.

How is on-call pay calculated for OR nurses?

On-call pay is standby hours times the on-call rate. As an example, 12 standby hours a week at $4.00 an hour for 52 weeks pay $2,496 a year. Your employer sets the rate, so use the figure in your own call policy.

Is on-call time counted as hours worked under federal law?

It depends on where you wait. The U.S. Department of Labor says an employee required to remain on call on the employer premises is working. An employee on call at home is not working in most cases, unless added constraints limit personal freedom.

What is call-back pay?

Call-back pay is pay for hours worked after you are called in outside your shift. This calculator multiplies your call-back hours by your base rate and by the multiplier you enter. The 1.5 default is an example, not a rule.

Does the BLS registered nurse wage include call pay?

No premium pay is included. BLS wages are straight-time gross pay, and the survey names overtime pay and shift differentials as excluded. This calculator keeps on-call and call-back pay out of the BLS comparison for the same reason.

What does the CNOR exam cost?

The CNOR application fee is $475, which includes the first exam attempt. A retake costs $175. The exam has 200 multiple choice questions and lasts 3 hours and 45 minutes, according to the Competency and Credentialing Institute.

What experience do I need for CNOR certification?

CNOR requires a current, unrestricted RN licence and current work in perioperative nursing. It also requires at least 2 years and 2,400 hours of perioperative experience, with at least 1,200 hours in the intraoperative setting.

Does the calculator add a premium for cardiac or neuro surgery?

No. No official source publishes pay by surgical service, so the tool has no service premium. Enter the base rate and call terms from your own offer, and the result reflects your service line.

Why does the calculator not show take-home pay?

Take-home pay depends on your tax filing status, state and deductions, and this tool does not estimate tax. It shows gross pay only, so that you can check each line against your pay stub.

Is my pay information saved?

No. The calculation runs in your browser and nothing is sent to a server. The Save button stores a result only on your own device.
